Patients
In order for patients to be enrolled in the ACTIMI Telecare program, they must meet the following criteria:
A heart failure of NYHA class II or NYHA class III
An ejection fraction of less than 40%
An ICD, CRT-P, or CRT-D pacemaker or a hospital stay due to chronic heart failure in the last 12 months
TMZ
To care for patients with chronic heart failure, you must register with your health insurance as a TMZ. We are very happy to support you in this process. The following conditions are required by the health insurance:
You are a specialist in cardiology
You are a doctor or medical care center authorized to participate in the statutory health insurance medical care according to § 95 SGB V
